Long Term Medical Conditions Annual Review

We are currently setting up a new recall system for all patients with one or more of the following chronic conditions:

CHD
(coronary heart disease)
Hypertension(high blood pressure)Diabetes
PAD
(peripheral arterial disease)
StrokeTIA 
Rheumatoid ArthritisAsthmaCOPD
(chronic obstructive pulmonary disease)

A number of the checks that are required for monitoring these conditions are similar. As such, in order to make appointments simpler we have grouped all of these conditions together under an umbrella title of  “Long Term Medical Conditions Annual Review”.

This will mean that where you have more than one of the above named conditions your appointment with the nurse will be to review them all at the same time. This hopefully will reduce the number of trips for review that each patient has to make through the year.

If you have any of the above listed conditions (apart from asthma or COPD) you will need to attend for a blood test with the Healthcare Assistant one to two weeks before your appointment with the nurse.

You will be contacted by the practice by letter, text or telephone, to arrange an appointment with a nurse for review.

For the first year you may find that you are called slightly earlier or later than expected.
