New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Please attend between 11.00am – 3.00pm to avoid busy times.
Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill, along with some photographic ID.
Practice Area
As a guide, the area covers Cleckheaton and Scholes and parts of Birkenshaw, Oakenshaw, Gomersal, Hartshead, Heckmondwike, Littletown, Liversedge and Roberttown.
Catchment Area
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration.

Online ‘Pre-Registration’ With The Practice
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the “Send” button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
Please note that we cannot complete your registration until you have come into Surgery with your ID and signed the form.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
Alternatively, you may print off a registration form, fill it out and bring it in with you on your first visit to the practice or complete the online form below.
Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
